TeleMetrix+ (TM+) is a medical grade, cloud-based Telehealth service that enables patients’ vital signs, such as blood pressure, blood sugar, including others such as weight to be measured and uploaded to our TM+ cloud-based repository from where it can be easily shared with medical professionals. TM+ is intended for use with selected outpatients who suffer from poor self-management of their own chronic disease conditions (hypertension, diabetes, COPD, heart failure) and would benefit from on-going (remote) supervision by their medical caregivers. TM+ is also useful for patients who have recently been discharged from hospital and whose recovery period (at home) would benefit from close follow-up, but without the need for time-consuming, face-to-face consults.
